Take the Mazzoni Center Survey

Mazzoni Center is looking for a few good men and women to take a new survey. The health facility would like to ask you about important health needs – and questions and concerns that most impact Philly’s LGBT community.

It’s quick. It’s anonymous. And – to make it even easier – it’s online.

And by pitching in, the center will be able to use your input to better serve the local community. This is your chance to not only talk about what you like about Mazzoni, but also any concerns about healthcare needs as a lesbian, gay, bisexual or transgender person living in the region.

Plus, when you take the survey, you can opt in to be entered to win a free gift card worth $100 (if you wish to enter the drawing for the gift card, the center will just need a first name and a way to contact you – but that information will not be attached to any answers you provide).
