The following is a raw report in Microsoft Excel format generated by the city’s Office of Food Protection containing descriptions of inspections carried out by them in 2004 and 2005. Where there is no description listed under the field titled CRITICAL_VIOLATION_TEXT, this indicates that the city's database does not contain any “critical violation data” for that restaurant on that date.

A report that appeared on this website earlier this month from the Office of Food Protection was found to contain erroneous information. The city has attributed this to an “IT problem.” They have assured us that the following information is correct. Download the file.
